We will liaise and work closely with you to identify and assess your needs and then create your care plan or package that reflects everything you need. 
We will then take it up with the relevant healthcare teams in the public and private sectors to ensure you are provided with the full range of services and funds. Also, you can opt to fund your care and support services or even top up the service you receive through your local Authority by direct payment or personal budget (depending if you qualify for funding from your local authority)
